In our community, many individuals who frequently call 911 for help may benefit from alternative resources that better meet their needs. Often, these calls are related to falls, lift assists, or recurring situations that could be more effectively managed with additional support.
Our Community Paramedicine Program was created in 2019 to address the needs of our department’s High-Use Group (HUG), who often require EMS services for non-emergency issues. Since its inception, the program has expanded to support all residents of Clay County, helping anyone in need access the right resources for their care.
Our compassionate team visits individuals in their homes to offer a free assessment of their situation. If desired, we work closely with each person to connect them with the most appropriate resources—helping to improve their independence and enhance their quality of life at home.
We can connect you with various free or volunteer resources designed to support your well-being. These include a growing list of local organizations, mental health services, medical care, and much more. Our team will stay in touch through regular follow-up visits, ensuring you receive the help you need until your situation improves.
If you or someone you know could benefit from this program, or if you’re an organization that would like to be included in our resource network, please email community.paramedicine@claycountygov.com or call (904) 529-4109 . We’re here to help you live your best life with the proper support at the right time.
Accessibility: Dial 711 (Telecommunications Relay Service) and ask the operator to connect you to (904) 529-4109 .
