Please complete and save the Sports Post-Event Report to your computer, or you can download and fill out the application by hand. Once you finish your application, be sure to gather all your documents and upload them here.

You can also submit your application and the accompanying documents by either emailing them to CCTourism@claycountygov.com or delivering them in person to the Clay County Tourism Visitor Center, located on the bottom floor of the Clay County Administration Building at 477 Houston Street, Green Cove Springs, Florida, 32221.
