E911 Addressing is committed to providing accurate and timely service to the citizens of Clay County. Our staff is responsible for assigning physical addresses for residential dwellings, commercial structures and accessory buildings in the unincorporated areas of the County; this includes features such as fountains and entry signs. In addition, addressing assistance is provided to the incorporated areas of the county as needed. To request an address, use the form below.
Our department is also responsible for approving new road names for the entire County. Developers may submit road names for consideration via email. Be sure to include the project name if applicable.
Approved Street Name Listing
This is a list of all road names that have been approved by the County. The Status field shows whether this road name is already in use or has been reserved for a future project. Please refer to this list when submitting new road names for consideration. Road names that duplicate an existing name will not be approved, even if the Street Designator is different. Online Application: The Address Application can not be filled out and submitted online.
Request for Address Form: Form can be printed and completed at home and faxed to Clay County Addressing at 904-541-5806 or mailed to P O Box 1366, Green Cove Springs, FL 32043. You may also turn it in at the Administration Building, 477 Houston Street, 3rd Floor, Green Cove Springs, FL 32043. Addresses cannot be issued at the time of delivery as most will require a site visit by County personnel.
Street Designator Definitions: This is a list of all approved street designators. Please refer to this list when deciding on the road name being submitted for consideration. If you are requesting a road name for reservation only, do not include a street designator. The designator will be assigned only after the road name is associated with a plat or if the street is being named as a result of an existing easement being named by the County.
